I could read the document, which is written both in old Venetian and latin, the correct name is MATHIO (you can find it also in the first record on top left page) repeated as MATHEI (latin version of Mathio = Matheus) in the genitive case in the record on the right bottom page. 

Indeed, it's an amazing document from Bale in Istria, it would be wonderful to find more documents concerning Garbin people in the area of Split and Hvar.
